METHOD FOR CARRYING OUT ASSISTED APPENDECTOMY IN VIDEOLAPAROSCOPIC WAY Russian patent published in 2005 - IPC

Abstract RU 2262897 C1

FIELD: medicine.

SUBSTANCE: method involves setting laparoscope at the level of upper umbilicus edge. The patient is arranged in head first position 20 mm large working port is set in blind gut cupula projection Surgical operation is carried out via single working port. Commissural process being available, auxiliary working port is set in ileocecal angle vicinity in left iliac region and soft intestinal forceps is introduced through the port. Surgeon grips blind gut tenia with the soft intestinal forceps near the cupula and makes a movement away from himself and upward in turning the intestine through 180°. Assistant surgeon fixes the appendix with the forceps allowing the surgeon to grip its apex. The surgeon retains the appendix in the port and the assistant surgeon draws laparoscope out and lets gas discharge from the abdominal cavity. The surgeon draws the port and appendix out from the abdominal cavity. Appendectomy is carried out. No way being available to expose the appendix in laparoscopic manner, ligature is brought under the appendix base. The ligature traction is carried out. The blind gut cupula and appendix base are pulled close to the port and placed on the anterior abdominal wall. Retrograde appendectomy is carried out. The blind gut cupula and appendix stump are immersed into the abdominal cavity. The working port is set at its initial place. Pneumoperitoneum is created. Abdominal cavity is healed and drained through the working port.

EFFECT: accelerated surgical intervention; reduced risk of traumatic complications.
