SUBSTANCE: the present innovation deals with surgical therapy of complicated ulcer of posterior duodenal all in patients with high degree of surgical risk as conditional-radical interference or in combination with antacid operations as a radical method of therapy. One should dissect the ulcer transduodenally, apply a П-shaped suture around ulcerous infiltrate, suture posterior duodenal wall with a ligature-holder, due to traction by a ligature and tightening П-shaped suture protrude posterior duodenal wall into its lumen, dissect the ulcer and suture posterior duodenal wall due to close technique with the help of a combined intestinal clamp that has got two curvatures of the branches that increase the angle of surgical action and enable to dissect the part of posterior wall out of length-restricted duodenotomy. On the clamp's working tips of tough fixation there are two clips for elastic maintenance of dissected and sutured duodenal walls. Moreover, the wound of posterior duodenal wall should be sutured in with one double-storied manual uninterrupted transfixion suture with atraumatic synthetic absorbable material. The innovation enables to simplify and accelerate the operation on dissecting the ulcer of posterior duodenal wall by implementing it according to close technique, decrease traumatism of the walls under suturing and provide prophylaxis of infecting the wound of posterior duodenal wall.
