METHOD FOR TREATING SURGICALLY THE ANEURYSM OF ASCENDING AORTA Russian patent published in 2008 - IPC A61B17/00

FIELD: medicine, cardiovascular surgery.

SUBSTANCE: it is necessary to lance aneurysmal sac longitudinally under conditions of artificial circulation and fulfill transverse incisions in its upper end without crossing the posterior wall of aorta. One should dissect aortic valve, at first, it is necessary to form posterior wall of distal anastomosis. Then one should cut a stripe out of xenopericardium, its length being equal to that of the segment of posterior aortic wall. With a blanket suture one should fix it towards posterior aortic wall so that its free end should face towards distal aortic department being at the level of anastomosis' suture line. Then with uninterrupted suture one should fix distal end of conduit, moreover, suturing ligature comes through the conduit's wall, xenopericardial stripe, aortic wall not entering its external surface. Then one should form anterior wall of distal anastomosis in the course of which it is necessary to suture with separate sutures as a trapezium the anterior wall of the conduit and anterior aortic wall. Then sutures should be tightened so that the conduit's edge should be in the lumen of ascending aorta, then sutures should be bound on interlayers out of xenopericardium, moreover, the width of suture upon interlayers equals to that on anterior wall of the conduit. The innovation enables to liquidate the risk of failed sutures, hemorrhage and create steady passage into anastomosis area.
