FIELD: medicine, cardio-vascular surgery.

SUBSTANCE: it is necessary to fulfill an access to mitral valve to dissect together with the affected subvalvular structures, suture the top of papillary muscle with a two-needle ligature 4-0 ePTFE, strengthen the suture with interlayers, tighten the ligatures toughly, guide both needles of the ligature mentioned through mitral valve's fibrous ring, guide ligatures from each papillary muscle through fibrous ring, match the desired length of ligatures to fix them due additional suturing the fibrous ring, tighten two ends of each suture and fix the prosthesis towards fibrous ring. Moreover, suturing the top of every papillary muscle should be fulfilled with one two-needle ligature 4-0 ePTFE, and through the fibrous ring one should apply two ligatures per one from each papillary muscle depending upon the location of papillary muscles in the cavity of left ventricle in pairs of points in position of 2 and 10 o'clock, or 3 and 9 o'clock, or 4 and 8 o'clock, at orientation of 12 o'clock for the middle of the bottom of anterior valve. The innovation enables to shorten the terms for reconstructing the bond between papillary muscle and fibrous ring, decrease the risk of error while detecting the length of artificial chordae.
