FIELD: medicine, perinatology.
SUBSTANCE: the present innovation could be applied in departments of intensive therapy and those of resuscitation and intensive therapy for premature neonatals. It is necessary to evaluate in points ranged 0-2 the results of clinical trials of the main body organs of systems against current normative parameters. Moreover, one should evaluate the following main organs and system of human body: central nervous system (CNS) (q1), respiratory system (q2), cardio-vascular system (q3), liver (q4), urinary-excretory system (q5), skin (q6) and, also, body temperature (q7). Moreover, while evaluating CNS 0 points is possible at the presence of normal reflexes, normal muscular tonicity in neonatals; 1 point - at muscular hypotonicity, hypodynamia, hyporeflexia, inert response to inspection; 2 points - at lost consciousness, muscular atonia, adynamia, areflexia; while evaluating respiratory system 0 points should be put if a neonatal does without oxygen; 1 point - if a neonatal needs oxygen through the oxygen mask or nasal catheter; 2 points - if a neonatal needs controlled ventilation or is upon spontaneous respiration at increased pressure at expiration through nasal cannulas or intubation tube; while evaluating cardio-vascular system 0 points - in case of normal HR, normal AP; 1 point - at moderate tachycardia (160-170 beats/min); 2 points - at pronounced bradycardia (<100 beats/min) or tachycardia (>170 beats/min), arterial hypotonicity; while evaluating liver 0 points is in case when liver isn't enlarged; 1 point - at increased liver by less than 2 cm; 2 points - at increased liver by more than 2 cm; while evaluating urinary-excretory system 0 points is observed at the presence of normal hourly diuresis; 1 point - at oligouria; 2 points - at anuria, hematuria; while evaluating the skin 0 points corresponds to norm; 1 point - at moderate paleness with perioral and acrocyanosis; 2 points - at pronounced jaundice, pronounced paleness, cyanosis, hemorrhage of diapedesis character; while evaluating body temperature 0 points is at normal body temperature (36.5-37.2°C); 1 point - at moderate hypothermia (36.4-36.0°C); 2 points - at hyperthermia (>37.2°C) or pronounced hypothermia (under 36.0°C). Then one should detect a premature neonatal's state severity degree by the following formula: q1+q2+q3+q4+q5+q6+q7=Q, where Q - total point; q1, q2, q3, q4, q5, q6, q7 - the quantity of points for each system or organ under inspection. Moreover, if Q value ranges 1-2 - this illustrates average-degree state in premature neonatals; 3-5 - severe state, 6-9 - highly severe state, 10-14 - extremely severe state. The innovation enables to objectivize the evaluation, detect the most vulnerable system in a child, based upon these data it is important to carry out certain volume of trials, prescribe adequate therapy in due time and predict the result of the disease, as well.
EFFECT: higher accuracy of evaluation.
