SUBSTANCE: one should make a crisscross incision of skin, subcutaneous fatty fiber, surface fascia, aponeurosis of gluteus maximus muscle followed by opening median layer of muscles and mobilization of the elements of lumbosacral plexus and sciatic nerve. Moreover, from crisscross projectional incision one should mobilize gluteus maximus muscle at the part from the site of fixation towards greater trochanter up to the site of fixation towards pelvic bones. The fibers of gluteus maximus muscle should be parted due to predividing them into portions and mobilized due to keeping the integrity of muscular fibers. Then it is necessary to isolate piriform muscle and mobilize it followed by mobilizing nervous trunks. The innovation provides decreased traumatism of operation due to keeping the integrity of muscular fibers.
