FIELD: medicine; surgery.

SUBSTANCE: separate a subcutaneously-fatty layer of a tissue by dissection, forming plural tunnels, find out and allocate perforans arterial, venous and lymphatic vessels, dissecting septum between the generated tunnels. Separate a mobilised dermal-fatty flap and deeper tissues round the allocated vessels with slantwise-vertical movements of the instrument jaws, serially changing location of the instrument and separation direction. Form the bridge-like flap including branch of a facial nerve, deep layers of a hypodermic fatty tissue and a superficial fascia. Make mobilisation of a flap of superficial muscularly-fascial system below the generated bridge-like flap. Dissect the mobilised flap of superficial muscularly-fascial system in a direction of an arch of a mandible from a corner level, dividing this flap on 2 parts. Pull the top flap under the generated bridge-like flap under a superficial leaf of temporal fascia, pull and fix it to a deep fascia above the bridge-like flap. Pull the bottom flap in a direction of the postaural area and fix it.

EFFECT: reduction of risk of squeesing of branches of a facial nerve and improvement of a lymphatic drainage from the mobilised tissues.
