FIELD: medicine; surgery.
SUBSTANCE: after the median laparotomy, liquidation of peritonitis source and primary sanation of abdominal cavity, mini-abdominal sections at 3-4 cm length are carried out on both left and right anterolateral and frontal abdominal wall. Flat drainages at width of 3-4 xm are put into mini-laparotomy cuts. The median laparotomy wound is covered by layers with napkins, separating purulent wound edges from abdominal cavity and leaving the abdominal cavity opened in the form of laparostoma. Scheduled sanations of the abdominal cavity through median laparotomy wound are carried out before the purification of its edges and decrease of intraabdominal hypertensia. Then a median wound is sutured in layers. The further sanations are carried out separately in abdominal cavity areas through mini-laparotomy cuts. In case of peritonitis and purulent liquidation in one of the areas the sanation is stopped. Sanitation of other areas is continued. Drainages are left before the termination of serous contents discharge from the abdominal cavity.
EFFECT: primary adhesion of median wound, decrease of sanations traumatism and increase of their efficiency, improvement of abdominal cavity drainage, acceleration of peritonitis liquidation.
