FIELD: medicine; traumatology and orthopaedics.
SUBSTANCE: canal is drilled in proximal clavicle splinter in direction from fracture plane towards sternal extremity and frontward until going out into soft tissues. Itramedullary canal is reamed in distal splinter until going out on posterior surface to acromial clavicle extremity. Intramedullary fixation with splinter compression by means of bar with thread on one end and flat widening with hole for screw on the other end is performed. Bar is introduced into hole in acromial clavicle extremity until its end goes out of distal splinter butt. Reposition of splinters is carried out. Bar is moved until flat widening of bar end is on bone, and its threaded end, made in form of truncated cylinder, appears in hole on anterior surface of clavicle. Cover plate with tube is put on threaded bar end which went out on anterior surface of clavicle, hole in tube being made for truncated cylinder. Screw nut is screwed on threaded bar end until compression of splinters is achieved. Bar locking is performed by passing screws, that have thread on heads coinciding with thread in holes, into flat widening on acromial extremity from back to front and into cover plate with tube from front to back. Device includes blocking screws, removable screw nut, removable cover plate with tube which is placed at 170 degrees to it, and metal bar with round transverse section, which has one threaded end for removable screw nut, and other end with flat widening, cover plate with tube is put on threaded bar end. Threaded bar end is made in form of truncated cylinder with truncation from both sides at 90° angle to said widening. Hole in tube is made for truncated cylinder to eliminate mutual mobility. Blocking screws are made with thread on heads, and threaded holes are made for heads of blocking screws in removable cover plate and said widening.
EFFECT: reliable fixation, prevention of rotary displacement of splinters, elimination of post-operation complications, shortening of treatment terms.
