METHOD FOR SURGICAL MANAGEMENT OF FRACTURE DISLOCATION OF ACROMIAL EXTREMITY OF CLAVICLE Russian patent published in 2013 - IPC A61B17/56

Abstract RU 2477982 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely traumatology and orthopaedics and aims at surgical management of the patients with fracture dislocations of an acromial extremity of clavicle. A surgical incision is made above an acromioclavicular joint and a distal end of clavicle. Injured and interpositioned tissues are removed from the joint. Clavicle fragments are drilled and repositioned. The fragments are fastened by an external plate which is screwed to the clavicle through the holes therein. A canal is drilled in an acrimoin. An acromial extremity of the clavicle is repositioned by means of an elastic fixation device comprising a loop and a cap with the cap having a size exceeding a diameter of a lower hole of the acromion canal. The loop of the elastic fixation device is brought bottom-up through the drilled canal in the acromion. A brought out loop of the elastic fixation device is dissected. Each end of the elastic fixation device is brought bottom-up through the two free holes at the end of the distal end of the plate. After the clavicle is repositioned, the ends of the elastic fixation device are strained together. The ends of the elastic fixation device are coupled together with its strained section placed parallel to the clavicle axis. The two free openings at the distal end of the plate are transversal to the plate axis. The coupling section of the section of the elastic fixation device is immersed in a groove between the two holes at the end of the distal end of the plate. A Lavsan tape of the elastic fixation device has a width of 3-4 mm. The elastic fixation device is made of a durably absorbable material, particularly of polysorb filaments.

EFFECT: method provides reduced postoperative joint dysfunction and intra-articular complications by eliminated bringing the element retaining the corrected clavicle through the joint.
