METHOD OF TERMINAL COLOSTOMY FORMATION Russian patent published in 2009 - IPC A61B17/00 

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to abdominal surgery, can be applied in treatment of large intestine diseases. On distal part of intestine, indenting 30-40 mm from machine stitch line, one or two cuts of serous membrane to muscular layer are made. Cuts are made at 5-7 mm distance from each other. 5-7 sero-serous interrupted sutures are applied circularly. Intestine wall in invaginated inward, forming circular valve-cuff. In staggered order transversal 5-10 mm long insections of serous-muscular membrane of distant part of large intestine are made at distance 20-30 mm from each other. Areas of serosomyotomy are located retroperitoneally. Retroperitoneal channel and opening are formed in anterior abdominal wall. Through opening prepared section of large intestine is brought out and fixed with interrupted sutures to skin. Valve-cuff is placed lower than level of aponeurosis of abdominal external oblique muscle. After 1 day intestine lumen is opened on line of machine stitch.

EFFECT: method allows to increase possibility of subjective control over act of colostomy emptying due to appearance of stable conditioned reflex in form of urge to empty distal part of intestine, ensures satisfactory function of continence, reduces peristomal dermatitis due to reduction of secretion of mucus, intestine juices on peristomal skin, reduces post-colostomy complications.
