METHOD OF TREATING CHRONIC OSTEOMYELITIS OF SHIN BONE COMBINED WITH DEFECTS OF SHIN BONES, INVESTING TISSUES, EXTENSIVE SKIN DISEASES AND DAMAGED ANTERIOR TIBIAL VASCULAR BUNDLE Russian patent published in 2010 - IPC A61B17/56 

FIELD: medicine.

SUBSTANCE: method involves sequestrnecrosectomy of an osteonecrosis centre, building of defect by a complex of pedicle tissues and fixation of limbs with an Ilizarov's compression distraction apparatus (CDA). The defect is built with a complex flap formed with using a radiation fasciocutaneous free vascularised flap from a forearm and not free vascular supplied foot flap from an opposite lower limb. Microvascular anastomoses are applied between arteries and two accompanying veins of the posterior foot flap of foot and the radiation fasciocutaneous flap. A pedicle of the radiation facsiocutaneous flap grafted into the formed tissue defect is wrapped with a not free posterior foot flap. It is followed with immobilisation of both lower limbs by applying the Ilizarov's CDA on both shins and a rigid unitising of the radiation and posterior foot flap herewith considering the length of the pedicles.

EFFECT: method provides preservation of the damaged limb and restoration of its support ability, elimination of osteomyelitis, possibility to built extensive defects of investing tissues of limb, improved blood circulation in the damaged segment of limb, reduced postoperative complications and treatment time.
