FIELD: medicine.
SUBSTANCE: invention relates to surgery and can be used for determination of tactics of treating suppurated epithelial pilonidal cyst (EPC) at abscess stage. Point index is calculated by summing up points obtained on the following criteria: age group younger than 30 years - 1 point, 30-40 years - 2 points, older than 40 - 3 points; previous treatment: treatment has not been carried put - 1 point, absence of effect from conservative therapy - 2 points, in case of disease recurrence after surgery - 3 points; abscess size: up to 2.0 cm in diametre - 1 point, from 2.1 to 3.5 cm in diametre - 2 points, more than 3.5 cm in diametre - 3 points; presence of secondary sinus tracts: one sinus tract - 1 point, two-three sinus tracts - 2 points, multiple sinus tracts - 3 points; by degree of purulent process spreading: strictly localised in sacrococcygeal region- 1 point, located with transfer on gluteal region - 2 points; located with transfer on gluteal and other regions - 3 points; by level of tissue pressure: less than 10 mm Hg - 0 points, from 10 to 15 mm Hg - 1 point, from 16 to 20 mm Hg - 2 points, from 21 mm Hg and higher - 3 points; by index of anatomic type of pelvis: narrow type of pelvis - 1 point, intermediate type of pelvis - 2 points, wide type of pelvis - 3 points. If point index is from 6 to 10 points indicated is lancing of abscess and EPC dissection with application of sutures in one step, from 11 to 15 points - indicated is lancing of abscess and EPC dissection, and after that, at the second stage of treatment, covering of wound defect at the expense of local tissues, in case of 16 and more points, indicated is abscess lancing and EPC dissection, and after that, at the second stage of treatment covering of wound defect with transplant.
EFFECT: method allows to increase objectivity in selection of treatment tactics.
