SUBSTANCE: invention refers to medicine, namely traumatology and orthopaedics. An operation is single-step from a single external approach to a hip joint. The first step involves transfemoral osteotomy of the femoral bone from an apex of a greater trochanter in a distal direction along the whole length of the femoral component with preserving a muscular pedicle on a dissected roof-like bone. Then the femoral component of the endoprosthesis is removed from the bone-marrow canal. The second step involves separating a neck, a head of the endoprosthesis from adhesions, an acetabular component from a pelvic cavity. After they removed, the formed cavity and the cotyloid cavity is filled with allografts that is followed by placing the exploring endoprosthesis and fixation of the dissected bone by wire sutures to the femoral bone.

EFFECT: method provides one-step endoprosthesis removal from one external approach, preserving lumboiliac muscles, adductors and greater trochanter, reducing length of operation, number of injures and postoperative complications, length of treatment.
