METHOD FOR CREATING INVAGINATED PANCREATOJEJUNOANASTOMOSIS Russian patent published in 2012 - IPC A61B17/11

Abstract RU 2465845 C1

FIELD: medicine.

SUBSTANCE: invention relates to surgery, and may be applied for creating invaginated pancreatojejunoanastomosis. A surface of a pancreatic stump section is coated with an adhesion surface of a flap of the hemostatic plate TachoComb cut by the shape of the stump section with a hole matched by position and size of a lumen of Wirsung duct. A bandage ligature is delivered on a distal segment of an intestinal loop at 0.5 cm from the section through a non-vascular segment of mesentery. A circular demucosation is performed at the distal segment of the intestine of the width of 0.5 cm at a mesenteric edge and 3 cm at an antimesenteric edge of the intestine. A continuous suture is applied between the mesenteric edge of the intestinal loop and a posterior surface of the mobilised segment of the pancreatic stump. Then 2 non-traumatic sutures 5/0 are applied on an anterior surface of the pancreatic stump at 2.5 cm from the edge; the same suture extends to suture an intestinal loop on the antimesenteric edge at 0.5 cm from the section; the pancreatic stump is invaginated in the intestinal loop. The bandage ligature is laid between the ends of the invagination sutures and tightened at graduated compression so that to bring an end of an artery clamp under. The bandage ligature is fixed by the ends of the invagination sutures as guided cantwise with respect to the pancreatic stump. The intestinal edge is fixed by surface sutures to a parietal peritoneum and a pancreatic capsule.

EFFECT: method enables improving leak tightness, reducing intraoperative injuries, reducing length of the procedure.
