FIELD: medicine.
SUBSTANCE: operation is performed under endotracheal anesthesia. Sampling of autotransplant, including posterior-lateral part of VIII rib from paravertebral line to rib arch on the side of planned formation of vascular anastomoses is performed. After that, transplant is brought to the place of defect. Distal rib fragment is fixed to upper jaw defect edges or to zigomatic bones. Proximal rib fragment is fixed to low jaw defect edges. Transplant revascularisation is performed by formation of microvascular anastomoses between intercostal and recipient vessels. After fixation of two-fragment bone autotransplant in zone of defects in buccal area from the side opposite to the side of formation of microvascular anastomoses, marked is point of intersection of vertical line, going from external corner of eye downward and line, drawn through corners of mouth. Common vascular pedicle is laid in arch way in the depth of soft tissues of buccal area, its lateral part being passed through the marked point.
EFFECT: method makes it possible to perform simultaneous full-scale reconstruction of complex combined defect of upper and lower jaws, provide reliable blood supply of autotransplant and thus increase predictability of results and reduce terms of complex rehabilitation of patients.
