METHOD OF TREATING PATIENTS WITH COMPLICATED CERVICAL SPINE INJURY Russian patent published in 2013 - IPC A61B17/56 A61B18/02 A61K33/14 A61P19/00 A61M25/01 A61N1/18 G01N33/48

Abstract RU 2475199 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely neurosurgery, and may be used for treating patients with complicated cervical spine injury. That is ensured by surgical intervention by spinal cord decompression, stabilization of the injured spinal motion segments, catheterisation of the epidural space and local hypothermia of the spinal cord. The local hypothermia of the spinal cord is performed by sodium chloride cooled to +5°C. To improve metabolic processes of nerve tissue and to avoid pyoinflammatory complications, a standard drug-induced therapy is prescribed from the first postoperative day. On the each 7th postoperative day, blood serum is examined by ELISA for matrix metalloproteinase MMP-2 and MMP-9, anti-neuronal immunoglobulin anti-MAG, phosphorylated neurofilament pNP-H, myelin basic protein MBP, tissue inhibitor of metalloproteinase TIMP-1 count. It is followed by calculating nerve tissue damage and nerve tissue restoration ratios by specific formulas. The derived values are estimated. Their total value is used to specify the following therapeutic approach. If the value is negative, the local hypothermia of the spinal cord is continued. If the value is positive, the local hypothermia is terminated; the catheters are removed; electrodes are punctured through interspinous spaces into the epidural space below and above the injured segments, and the spinal cord is electrically stimulated.

EFFECT: method provides higher clinical effectiveness, reduced risk of postoperative complications, earlier starting date of rehabilitation measures ensured by choosing an individual therapeutic approach.
