FIELD: medicine.

SUBSTANCE: corneal tunnel and two paracenteses are formed in the direction of 11 o'clock or 1 o'clock. A viscoelastic is introduced into an anterior eye chamber through the corneal tunnel. Spatulas are used to dilate a pupil, and a capsulorrhexis is performed. A hydrodissection is performed. A chopper is used to rotate a nucleus in a capsular sac. Cortical and epinuclear layers are sheared within the casulorrhexis. A nucleus of lens is separated. A first furrow is made with a phacopoint from the corneal tunnel in the direction of 1 o'clock or 11 o'clock. A second furrow is made with the nucleus unturned at an angle of 30-70 degrees to the first furrow in the direction of 4 o'clock or 8 o'clock respectively. The furrow length extends to the edge of the capsulorrhexis. Branches of the capsulorrhexis forceps are inserted into the formed furrows. A first fragment found between the furrows is chipped from the nucleus of lens. A capsulorrhexis forceps tip is turned at 90 degrees, and the first and second furrows are extended to the edge of the capsulorrhexis. The branches are inserted into the formed furrows. They are dilated until the second and further fragments separate from the nucleus of lens. A phacoemulsification covers the second fragment first and then the rest fragments in the centre of the capsular sac.

EFFECT: effective and safe phacoemulsification in the given category of patients, simplified further manipulations without loosening of tendons of Zinn.
