FIELD: medicine.
SUBSTANCE: extra-oral device for anatomical positioning of mandibular bone fragments comprises a guide, pins and supporting arms for pin retention. The guide is presented in the form of two arc fragments retained together by a movable supporting arm with two locking nuts, and each pin represents a hollow tube. An intraosseous portion of the tube comprises a bevel inclined to a long axis at 45° and 8 1-mm drug-feeding access holes. An outer surface of the intraosseous portion of the pin is threaded roughly at a screw pitch of 0.75 mm over a length of 20 millimetres. An extraosseous portion ends in a flattened grooved surface. The supporting arm used for pin retention is demountable and consists of inner and outer bolted fragments. The outer fragment comprises two mutually perpendicular access holes; from its inside, a horizontal hole is silicone-packed sealant and configured to accommodate the pin, while a vertical hole is used for the threaded pin retainer. A receiving surface for guide arc retention is formed on the inner surface. The guide arc is configured in accordance with multi-layer computed tomography findings by the prototyping process of stereolithography.
EFFECT: invention enables improving the clinical effectiveness in bisphosphonate osteonecrosis ensured by the anatomical positioning of the mandibular bone fragments with making it possible to prevent and treat this disease in the future.
