METHOD FOR SURGICAL MANAGEMENT OF THYROID DISEASE Russian patent published in 2015 - IPC A61B17/00

Abstract RU 2545441 C1

FIELD: medicine.

SUBSTANCE: neck skin, subcutaneous fat and muscles are incised on the side. If approaching from the left, the incision starts from an anterior edge of trapezius muscle and extended to an outer edge of sternocleidomastoid muscle. If approaching from the right, the incision starts from an outer edge of the sternocleidomastoid muscle and extended to an anterior edge of trapezius muscle. Short neck muscles are dissected. Thyroid lobes are mobilised from the side surface. Recurrent laryngeal nerves are inspected visually and electrophysillogically through the neck starting from posterior triangle of neck up to an entry into laryngeal constrictors. Paratyroid glands are separated together with a vascular pedicle. Upper and lower thyroid arterial trunks are ligatured within a bifurcation. The bed is drained through a corner of the same incision. The wound is closed in layers.

EFFECT: method enables avoiding cicatrisation on the neck, injuring the recurrent laryngeal nerves and parathyroid glands due to the optimal incision, and the visual and electrophysiological inspection and drainage of the bed through the edge of the same incision and also provides a intraoperative view of the anatomical structures, the absence of excessive skin traction due to dissection of the short neck muscles.
