FIELD: medicine.

SUBSTANCE: invention relates to the field of clinical laser medicine and can be used in carrying out transmyocardial laser revascularisation of the myocardium (TMLR) both separately and in a combination with aorthocoronary bypass surgery (ACBS). A device for the realisation of transmyocardial laser revascularisation (TMLR) includes: a laser source of radiation, representing a solid-state laser based on neodymium-doped yttrium aluminium garnet, working at a wavelength of 1.44 mcm, a control and indication unit, connected to the laser source of radiation, a fibre-optical system, which has a reciprocating mechanism of optic fibre supply and intended for the transportation of laser radiation into the zone of carrying out TMLR, and a terminal device containing a replaceable cassette with a segment of an optical fibre and disposable syringes and having an output hole for the optic fibre, with the device being provided with a rate sensor, connected to the control and indication unit by means of feedback, a floor pedal for launching the process of laser revascularisation; the replaceable cassette is connected to the terminal device by means of magnetic connection, with an end of the optic fibre being placed inside the replaceable cassette with a clearance to the output hole, with the device itself being made with a possibility of a laser pulse impact in case of both direct and reverse movement of the optic fibre.

EFFECT: increased quality and efficiency of TMLR and TMLR in the combination with ACBS, reduced risk of heart complications in the operative and postoperative period.
