FIELD: medicine.
SUBSTANCE: invention may be used to diagnose an oesophageal neoplasm expansion by magnetic resonant tomography (MRT). Integrated MRT is performed before treatment and during controlling the results of chemo- and/or radiation therapy. The study is three-staged. The first stage involves forming high-resolution T2-weighted images in axial and sagittal planes with using a pulse sequence of fast spin-echo with the following parameters: field of view (FOV) 32, slice thickness 4.0, train repetition (TR) period 675, echo signal time (TE) 100, array of frequency and phase 384 and 256 respectively, echo train length (etl) 23, number of excitations (NEX) in the layer selected 4. The formed images are used to talk about the neoplasm expansion, oesophageal layers differentiation, their relation to surrounding organs and tissues, as well as on the presence of metastatic changes and doubtful sites of metastatic involvement to be visualised by means of the second stage of MRT. To this effect, at the second stage diffuse weighted images (DWIs) are formed in the axial plane with the following pulse train parameters: B-value 800, field of view 36, slice thickness 5.0, train repetition (TR) period 8,000, minimum (min) echo signal time (TE), array of frequency and phase 128 and 192 respectively, number of excitations (NEX) in the layer selected 16. Doubtful and metastatic sites are visualised on the T2-weighted images by mapping the formed DWIs onto the T2-weighted images. The third stage involves forming T1-weighted images in the signal suppression mode from fatty tissue before and after administering a gadolinium-containing contrast medium, with using a fast clear gradient-induced echo train with the following parameters: field of view (FOV) 32, slice thickness 4.0, train repetition (TR) period 370, minimum (min) echo signal time (TE), Flip angle 80, array of frequency and phase 384 and 384 respectively, echo train length (etl) 23, number of excitations (NEX) in the layer selected 2. A degree of contrast medium accumulation shows a degree of tissue involvement into the neoplasm.
EFFECT: more informative diagnosing of this pathology by using an integrated approach to the examination, improving MRT visualisation characteristics, and optimising examination parameters.
