FIELD: medicine.
SUBSTANCE: invention relates to medicine, methods of neurovisualisation for determination of indications for intravenous thrombolytic therapy (TLT) in acute period of ischemic stroke (APIS). Patient is subjected to magnetic-resonance tomography (MRT) of brain in mode of diffusion-weighted imaging (DW-MRT) by means of diffusion-weighted echo-planar pulse sequence: TR = 4300 ms, TE = 119 ms, averages = 3, dist. factor (gap) 0%, FOV = 250 mm, matrix 128×128 pixel, slice thickness = 3 mm, b-value 0; 500; 1000 s/mm2 for the first 4.5 hours from the moment of neurologic symptomatology development. In case of MRT-signs of acute ischemic injury of brain are visualised additional MRT of brain is performed in mode of T2-weighted imaging (T2*-WI) by means of pulse sequence gradient echo, with parameters: TR' = 800 ms, TE = 26 ms, averages = 2, dist. factor (gap) 0%, FOV = 250 mm, matrix 168x256 pixel, slice thickness = 3 mm, flip angle=20 deg. Examination is carried out in axial plane, whose cuts are parallel to the line between lower edge of hypophysis and fundus of the fourth ventricle, which makes it possible to compensate artifacts from hyperpulsation of cerebrospinal fluid in subarachnoid spaces influencing image quality. If MRT signs of intracranial hemorrhage are absent, TLT therapy is considered indicated.
EFFECT: method has high specificity and sensitivity in respect to verification of cerebral ischemia, elimination of intracranial hemorrhage and determination of indications for TLT.
