SUBSTANCE: invention relates to medicine, specifically to surgery of inguinal hernias. Anchoring lower edge of abdominal internal oblique and transverse muscle of abdomen with a transverse fascia to iliac-pubic tract. Then lower edge of graft is sutured to inguinal ligament. Hole is opened for spermatic cord by taking clamp in fold mesh precisely above centre of internal ring, formed by shears hole for spermatic cord. Upper border of mesh from inside to aponeurosis of external oblique muscle uninterrupted suture to point of dissection. Uniform stretch mesh in inguinal space by lap and further excision of excess lateral flap mesh. Integrity restoration of mesh is performed by continuous suture to form a window for spermatic cord of required size.

EFFECT: intraoperative cutting half fixed graft enables accurate matching internal inguinal ring and opening in graft, which enables to avoid developing early postoperative complications and recurrent hernias.
