METHOD OF DETERMINING REHABILITATION POTENTIAL IN PATIENTS WITH STABLE COMPRESSION FRACTURES OF THORACIC SPINE Russian patent published in 2016 - IPC A61B5/22

FIELD: medicine.

SUBSTANCE: invention relates to restorative medicine and can be used to determine rehabilitation potential (RP) of patient with compression stable fractures of thoracic spine. Neurological examination is performed. First, spinal motor functions are estimated in 6 segments from both sides - muscle strength of: T1 - abductors of 5-th finger, L2 - femoral flexors, L3 - knee extensors, L4 - rear foot flexors, L5 - thumb extensors, S1 - plantar foot flexors. Complete paralysis is evaluated as 0 points, palpable or visible muscle contraction as 1, active motion (AM) in easier position as 2, AM in normal position as 3, AM with overcoming of moderate resistance as 4, AM against full resistance as 5 points. RP determinant value for motion sphere is found on total sum. If sum is 57-60 points, disorders are absent, RP value = 0; 45-56 - weak disorders, RP = 1; 30-44 - average disorders, RP = 2; 3-29 - intensive disorders, RP = 3; 0-2 - full disorders, RP = 4. Then sensitivity in 21 segment is assessed at both sides: T1 - medial side of ulnar fossa, T2 - armpit top, T3 - third intercostal space, T4 - level of nipples, T5 - 5th intercostal space, T6 - 6th intercostal space, T7 - 7th intercostal space, T8 - 8th intercostal space, T9 - 9th intercostal space, T10 - navel level, T11 - 11th intercostal space, T12 - inguinal fold, L1 - half distance between T12 and L2, L2 - front middle of femur, L3 - medial condyle of femur, L4 - medial ankle, L5 - rear surface of foot at level of 3d metatarsophalangeal joint, S1 - lateral surface of heel, S2 - popliteal fossa along the mid-line, S3 - ischial tuberosity, S4-5 - perianal area. Sensitivity in these zones is assessed by points: absence of sensitivity is 0 points, violated is 1 point, normal is 2. RP determinant value for sensitivity is found on total sum of points. 80-84 points - sensitive disorders are absent, RP = 0; 63-79 - weak disorders, RP = 1; 43-62 - average disorders, RP = 2; 4-42 - high disorders, RP = 3; If total sum is 0-3 points, full disorder is diagnosed, RP = 4. Sum of RP values of motion and sensitive spheres is determined, and if total RP = 0, it is considered, that RP is very high, 1-2 - high, 3-4 - medium, 5-6 - low, 7-8 - very low RP.

EFFECT: method provides quantitative assessment of RP in patients of this group, which has predictive value and allows to prepare rehabilitation program in required volume.
