FIELD: medicine.
SUBSTANCE: anti-ischemic kidney protection is performed in the limb-surgical treatment of patients with local kidney cancer under conditions of warm ischemia. The standard preparation surgery procedure and anesthetic management are performed. Herewith the epidural blockade is performed after premedication by continuous injection of the ropivacaine mixture of 2 mg/ml and epinephrine of 2 mg/ml at the rate of 6-10 ml/hour into the epidural space by using a syringe metering pump or an elastomeric micro-dose pump throughout the operation, then the general anesthesia and the kidney resection are performed by using laparoscopy under the conditions of warm ischemia. Also, the epidural blockade is performed during the first three days of the post-operative period.
EFFECT: expressed nephroprotective effect during the LPN under the conditions of warm ischemia of up to 25 minutes, protecting the kidney from ischemic injury, the ability to reduce the hospitalization time and to reduce the economic treatment costs of acute kidney injury.
