SUBSTANCE: invention relates to medicine, in particular to cardiology, interventional arrhythmology and cardiac surgery. Method for determining an electroanatomical substrate in high density contact mapping of the endocardial surface of the left atrium to stratify the risk of atrial fibrillation recurrence after pulmonary veins circular isolation in patients with paroxysmal and persistent atrial fibrillation include high-density contact mapping and a three-dimensional reconstruction of the left atrium. Construction of the left atrium amplitude and activation charts is performed by sequential reading of the amplitude and activation information upon contact of the ablation-mapping electrode from the surface of the left atrial endocardium at the correct points, where the distance of the point from the reconstruction surface does not exceed 1 mm, standardized both over the distance between adjacent points and along the distance from the tip of the electrode to the endocardial atrial surface. Based on the obtained results, the cardiac zones of the lowered voltage in the spectrum of the amplitudes ≤ 0.75 mV, associated with a slowing down of the electric pulse carrying speed of less than 1 m/s are discovered, and which are an electroanatomic substrate. Relative area of the electroanatomic substrate of the left atrium is measured as a percentage, in accordance with the value of which the relative risk of atrial fibrillation recurrence is calculated after circular isolation of the pulmonary veins in accordance with the stratification formulas and the calculated stratification charts of the risk of atrial fibrillation recurrence.
EFFECT: invention makes it possible to expand the functionality of the left atrium endocardial surface electroanatomical mapping without distorting of the endogram amplitude parameters.
