SUBSTANCE: invention relates to medicine, namely, to the surgery of the paranasal sinuses. Cut out the bone-plastic flap in the region of the wall of the paranasal sinus as a single complex fragment. Bone-plastic flap is either broken and bent, or completely removed. They open the sinus. Remove the pathological contents of the sinus. Wash the wound. Before placing the flap in the lumen of the sinus, the patient prepared from the patient's pre-taken blood is placed over the area of the bone defect by horizontal centrifugation of a plasma clot enriched with fibrin. Edges of the clot should extend beyond the flap, and part of the clot should protrude into the bone wound. Fix nodal sutures with a single block with the mucosa of the flap and the operated sinus, passing the needle through the soft tissues of the flap, the clot and through the soft tissues of the edges of the wound.

EFFECT: method makes it possible to perform osteoplastic surgery on the paranasal sinuses, to prevent postoperative complications due to the use of plasma enriched with fibrin, as well as reliable fixation.
