FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely, to ophthalmology, and is intended for predicting the safety of surgical interventions in relation to the progression of aniridic keratopathy (AK) in patients with congenital aniridia, depending on the molecular mechanisms of damage to the PAX6 gene. To predict the safety of surgical treatment of patients with keratopathy associated with congenital aniridia, affecting the limbic stem zone of the cornea, the result of the genetic analysis on the verification of the mutation in the PAX6 gene is taken into account. When a chromosomal rearrangement is detected, an exciting 3'cis-regulatory region and located at the level of the ELP4 and DCDC1 genes, biomicroscopic confirmation of the 0 AK stage, the presence of progenitorial limbal structures according to data of laser scanning confocal microscopy (LSCM) and optical coherence tomography (OCT) carrying out surgical interventions in the limbal zone is associated with a minimal risk of developing corneal decompensation and loss of its transparency. If there are other mutations in the PAX6 gene in combination with the AK pattern, alteration of limbal progenitor structures according to LSCM and OCT, surgical interventions are associated with a high risk of progression of limbal deficiency, leading to the formation of an abnormal reparative response, conjunctivization of the cornea, loss of its transparency and a significant decrease in visual functions.
EFFECT: use of the invention makes it possible to prevent or reduce the risk of progression of AK in the early and late postoperative periods, as well as maintain the transparency of the cornea, visual acuity and reduce the number of subjective complaints.
