FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to cardiovascular surgery, and can be used in treatment of heart valve defects, particularly with annuloplasty of the mitral valve. According to the invention, the ring framework for annuloplasty of the mitral heart valve is made of titanium nickelide in the form of an open saddle ring of round wire with a diameter of 0.2–1.0 mm. Both ends of the frame are formed as a closed loop facing outwards. Method of manufacturing an open ring framework for annuloplasty of the mitral valve of the heart includes building a computer model of the framework, molding and polishing. First, from the preform for the frame, an open contour is formed and closed loops are formed at the ends with the aid of a first forming matrix having, on the cylindrical base, a rod for winding the wire and two cylindrical pins for forming loops and removable clamping plates. Preform for the skeleton fixed on the matrix is heated in a vacuum oven, followed by cooling in water. Then, the skeleton is spatially shaped with the aid of a second shaping die consisting of pressing members and a central block with a horizontal groove formed on the outer surface for filling the carcass preform, while the geometry of the central block and the groove repeat the shape of the fibrous ring of the mitral valve, the preform fixed on the matrix is heated in a vacuum oven, then cooled in water. Then the frame is removed from the matrix and polished, and passivated. After giving the nitinol frame the final spatial shape, it can be used as an independent product for implantation or subjected to lining with bio- and hemocompatible materials. Use of two form-building matrices allows us to achieve the necessary elastic-deformative properties of the framework and to create an exact spatial shape that persists at temperatures of 34–42 °C. Formed in the form of a closed loop and outwardly exposed ends of the frame, during the implantation, the joint contour of the joints can not be moved inside.
EFFECT: creation of an open ring framework (C-ring) for annuloplasty of the mitral valve of the heart, whose shape corresponds to the spatial form of the natural mitral fibrous ring, as well as a decrease in the perimeter and area of the dilated fibrous ring as a result of fixation of the annuloplastic implantable skeleton, and minimization of the number of foreign materials in the heart cavity.
