FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to surgical dentistry. Perform procedures for exfoliation of the mucoperiosteal trapezoidal flap, opening the compact outer lamina of the alveolar process of the maxilla, formation of the bone bed for a bandage implant and perforation of the maxillary sinus floor, exfoliation of the mucous membrane of the sinus floor from the underlying bone base, filling the entire volume of the resulting cavity with a liquid osteoplastic preparation, and installing a screw implant in the formed bone bed. Procedure for installing the intraosseous part of the screw implant is carried out directly after the formation of the bone bed. Moreover, the intraosseous part is made with a through, threaded, axial hole, with longitudinal grooves along its external thread and with an angle of inclination of the grooves of the thread to the longitudinal axis. Perforation of the bottom of the maxillary sinus is performed with a device containing a bone trephine with an atraumatic drill on one and a manual wing on its other end, which is placed in a through-hole, axial, threaded hole of the intra-bone part. Exfoliation of the mucous membrane of the bottom of the sinus from the underlying bone base with simultaneous filling of the emptying cavity with liquid osteoplastic material is performed by the device, which is installed in the through, axial, threaded hole of the intraosseous part, containing a threaded rod with a smooth hemisphere at one end, and a through axial hole, passing into a conical hole fitting with a syringe with osteoplastic material located in it.
EFFECT: method allows to exclude multiple installation and removal of various devices, each time injuring the walls of the bone hole, excluding its additional infection and weakening of the mechanical properties of the tissues of the bone bed; proposed design of the device “elevator-choke” will allow combining the procedures for exfoliating the mucous membrane of the sinus floor from the underlying bone base with simultaneous filling of the emptying cavity with liquid osteoplastic material, thereby minimizing the probability of losing a certain amount of expensive osteoplastic material and clogging the surgical field.
