FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely, surgical dentistry, and is intended for use in prosthetics on implants with a significant atrophy of the alveolar process of the maxilla and a decrease in its height in the distal sections. Under local anesthesia on a edentulous area of the alveolar process of the upper jaw, mucus-periosteal trapezoidal flaps are cut and peeled off. Compact outer plate of the alveolar process of the upper jaw is opened. Then, a bone bed of the screw dental implant is made directly up to the wall of the bottom of the maxillary sinus. After that a screw implant containing the intra-osseous part is installed in the produced bone bed, with a large thread on its outer surface with an angle of inclination of the thread carvings to the axis, on top of which, in addition, a fine thread is formed. On the outer surface of the intraosseous part and at the same angle, longitudinal grooves with a length of max. 3/4 of its height are made. Through the axis of the intraosseous part of the implant a through threaded hole is made. Then, in the axial threaded hole of the intra-osseous part, a "trephine-perforator" device is screwed in, containing a bone trephine with a smooth cylindrical shank and a manual nut on its end located in the through axial aperture of the support screw with a manual nut at the end. Perforation of the bottom wall of the maxillary sinus is performed manually, by rotating the bone trephine, using the manual nut. Moment of the final perforation is marked by a light dip along the axial movement of the trephine. Then, this device is unscrewed and in its place an "elevator" device is installed to perform the exfoliation of the mucous membrane of the bottom of the maxillary sinus from the underlying bone base. This device, located in the threaded axial hole of the body of the intraosseous part, contains a threaded rod with a smooth hemisphere at one end and a manual nut on the other. In this case, the rotational movement of the manual nut will coincide with the forward movement of the smooth hemisphere into the depth of the maxillary sinus. Further, the procedure of filling the entire volume of the formed cavity with a liquid solution of a collagen gel is carried out. This procedure is carried out by a device which is also installed in the threaded axial opening of the body of the intraosseous part. This device is made in the form of a socket with an external thread and a through axial cylindrical hole, passing into the conical hole, where a syringe with the above-mentioned gel is located. Thus, under the pressure of the piston, the collagen gel from the syringe enters the empty cavity of the maxillary sinus, gradually and completely filling it. At the end of the final procedure, the screw implant is finally placed in the required position, and the Metapex drug is inserted into its threaded axial hole and a plug is inserted. At the end of the surgery the wound of the periosteum and mucous membrane is stitched using a hermetic suturing.
EFFECT: method allows to reduce surgical injury and increase the durability of the soft sinus lift procedure in combination with single-step implantation.
