SUBSTANCE: invention relates to traumatology and orthopedics and may be applicable to the surgical treatment of periprosthetic infection of large joints. Separate the metal endoprosthesis and remove its head and polyethylene liner. Wash the surface of the wound cavity and exposed surfaces of the disconnected metal endoprosthesis with a 3 % povidone-iodine solution using a pulsatile lavage system. Wash the surface of the wound cavity and the exposed surfaces of the disassembled metal endoprosthesis with a 1 % lavatept solution using ultrasound lavage. They place the new head of the endoprosthesis and a new polyethylene insert in the metal endoprosthesis and restore the integrity of the endoprosthesis. Wash the surface of the wound cavity and the surface of the assembled metal endoprosthesis with a 1 % lavatept solution using ultrasound lavage. On the surface of the wound cavity and on the surface of the assembled metal endoprosthesis, a gel with bacteriophages “Phagodent” evenly distributing it along the entire surface of the wound cavity and the surface of the assembled metal endoprosthesis, providing a thickness of the gel layer from 1 to 3 mm. Then, the wound is closed. Within 3–5 days of the postoperative period, the wound is drained with a 1 % solution of the bacteriophage “Phagodent” preparation from the bottom up through the perforated drains.

EFFECT: method allows to preserve the endoprosthesis, to reduce the risk of reoperative surgery on the affected joint, to restore the functional capabilities of the limb.
