METHOD FOR OPERATIVE TREATMENT OF UROTHELIAL CARCINOMA OF MIDDLE THIRD OF LEFT URETER Russian patent published in 2019 - IPC A61B17/00

SUBSTANCE: invention refers to medicine, namely oncourology. Laparoscopic segmental ureterectomy is performed, in which resection of the affected part of the ureter is performed with subsequent restoration of its integrity. Time access to the ureter is carried out through an incision in the mesentery of the descending colon, limited by the branches of the left colonic artery and the inferior mesenteric vein. Upper limit of resection of the ureter is carried out 2–2.5 cm below the pelvic-ureteric segment. Lower limit is performed at the level of the common iliac artery. Replacement of the lost ureteral fragment is performed by an unreconfigured segment of the small intestine, the length of which corresponds to the ureteral diastasis and which is isoperistaltic and located between the upper and lower ends of the ureter. In this case, the anastomosis of the upper part is carried out according to the type “end of the ureter – to the side of the intestine”, the lower end is anastomosed with the neo-ureter, like “end of the intestine – to the end of the ureter”.

EFFECT: method provides the ability to save the kidney, saving the patient from urothelial carcinoma of the middle third of the left ureter with minimal oncological risks, minimizes the risk of injury to the intestine and spleen, eliminates the development of complications associated with these injuries, eliminates possible damage to the ureter and the risk of insolvency and / or anastomotic stricture, reduces the time of surgery, allows to keep urodynamics as close as possible to the physiological state in compliance with all norms of ablastics.
