SURGICAL INSTRUMENT FOR CUTTING AND STAPLING TISSUE Russian patent published in 2019 - IPC A61B17/68 A61B17/72

Abstract RU 2678365 C1

FIELD: medicine.

SUBSTANCE: group of inventions relates to medical equipment, specifically to a surgical instrument for cutting and stapling tissue. Surgical instrument for cutting and stapling tissue comprises an elongated shaft assembly, a surgical end effector and means for simultaneous connection of the proximal end of the first portion of the end effector with the elongated shaft assembly, an articulated connecting element with a hinge assembly and a drive assembly to the proximal firing element. Elongated shaft assembly comprises a proximal firing element and a proximal hinge assembly. Proximal firing element operably interacts with a firing system to receive firing and retracting movements therefrom. Proximal hinge assembly operably interacts with the articulated joint system to receive pivot movements therefrom. Surgical end effector comprises first and second end effector portions, an articulated joint, an articulated connecting element assembly and a drive assembly. First portion of the end effector comprises a carrier configured for functional support of a cartridge with surgical staples therein. Second portion of the end effector comprises an anvil assembly and is selectively movable relative to the first portion of the end effector in a first direction upon application of an opening movement to the second portion of the end effector and the second portion of the end effector further is selectively movable in a second direction upon applying a closing movement to the second portion of the end effector. Articulated joint is adapted to selectively pivot the first portion of the end effector about the axis of articulation in the first and second directions relative to the longitudinal axis of the instrument. Articulated connecting element assembly operably interacts with the articulated joint for applying movement to the articulated joint. Drive assembly is supported in the surgical end effector for functional movement upon application of firing and retracting movements thereto, wherein the drive assembly is configured to apply closing and opening movements to the second portion of the end effector. Drive assembly comprises a cutting head comprising a vertically extending portion, a lower leg, an upper portion of a tab and the actuator rod assembly. Vertically extending portion includes an upper end and a lower end and a tissue cutting portion oriented between the upper end and the lower end. Lower leg protrudes from the lower end of the vertically extending portion and is configured for mobile engagement of the lower portion of the carrier. Upper tab portion protrudes from the upper end of the vertically extending portion and is configured to engage the portion of the anvil assembly. Drive rod assembly is operatively connected to the cutting head and comprises an upper segment of a firing rail and a segment of a lower firing rail. Segment of the upper firing rail is connected to the upper end of the vertically extending portion of the cutting head. Segment of the lower firing rail is connected to the lower end of the vertically extending portion of the cutting head. Upper and lower firing rails are pivotally connected to the proximal engagement element. Disposable loading unit for attachment to the elongated shaft assembly of the surgical instrument, wherein the disposable loading unit comprises a disposable housing assembly, a carrier, an anvil, an articulated joint, an articulated connecting element assembly and a drive assembly. In the second embodiment, the surgical instrument comprises an elongated shaft assembly, a surgical end effector and means for simultaneously connecting the distal end of the first portion of the end effector with the elongated shaft assembly, a hinge connecting element with a proximal hinge assembly and the drive assembly. In the second embodiment, the disposable loading unit for attaching an elongated shaft of a surgical instrument to the assembly comprises said housing assembly, carrier, anvil, articulated joint, articulated connecting element assembly and drive assembly. Drive assembly is configured for application of opening and closing movements to the anvil and comprises a cutting head, a proximal engagement element, a segment of the upper firing rail and a segment of the lower firing rail.

EFFECT: group of inventions is characterised in that each of the upper and lower firing rails of the drive rod assembly of the cutting head is pivotably connected to the proximal engagement member.
