FIELD: medicine.
SUBSTANCE: invention relates to the field of medicine, namely to an angiographic bronchial catheter for selective catheterization of the bronchial and intercostal arteries with a transvenous access through a defect in the interventricular septum and a method for performing selective catheterization of the bronchial and intercostal arteries with the transvenous access through the defect in the interventricular septum. Catheter is made in the form of an elastic tube with a first end straight working section, conjugated with a first bend with curved sections of the catheter tube, forming sections of a hyperbola or parabola, depending on a modification of the catheter. Base of the elastic tube is oriented at an angle of 20° to the axis of symmetry of a parabola branch and 20° to the real axis of the hyperbola branch of the working end of the catheter, which form the bends of the tube, repeating the shape of the aortic arch. First section of the catheter has a conical part of 8–10 mm long with atraumatic rounding from the end. Second and third bends are made in the form of elastic forming elements. Method includes performing transvenous endovascular catheterization of the right heart cavities using the angiographic catheter, which is then carried out through the defect in the interventricular septum into a lumen of the ascending thoracic aorta and further into the aortic arch. Through a diagnostic catheter into the lumen of the aortic arch and further into the descending aorta, a hydrophilic atraumatic guide is carried out. Diagnostic catheter is removed, and a bronchial catheter is inserted into the area of the thoracic aortic isthmus through the established hydrophilic atraumatic guide. Guide is removed, the first working section of the catheter, due to its own elasticity, assumes a predetermined shape, whereby it slides off into a desired mouth of the bronchial artery. Then perform selective bronchial arteriography.
EFFECT: decrease in irradiation and the amount of radiopaque substance introduced into the bloodstream.
