METHOD OF LAPAROSCOPIC RADICAL NEFREKTOMY WITH THROMBECTOMY FROM THE VENA CAVA INFERIOR Russian patent published in 2019 - IPC A61B17/00

SUBSTANCE: invention relates to medicine, namely to abdominal surgery and urology, and can be used in surgical treatment of kidney oncological decease – clear-cell carcinoma complicated by thrombosis in vena cava inferior (VCI). To do this, under X-ray control through jugular vein into the vena cava inferior below the hepatic veins using endovascular set unsolved thromboxanes and reveal it at the apex of tumor thrombus. After that, through the subclavian vein or another jugular vein, an undisclosed balloon catheter is installed at the level of hepatic veins, but above the thrombus. Trocars are installed, mobilize the kidney to be removed and cut it off. Then do cavataio and thrombectomy with resection of a part of the wall of the vena cava inferior suturing of the defect of the vena cava inferior. Remove the cut kidney and tumor thrombus with a part of a resected wall of the vena cava inferior. After cutting off the kidney the hepato-duodenal ligament with the portal vein is isolated, followed by a temporary overlay of turnstile around the ligament.

EFFECT: method ensures safety of laparoscopic radical thrombectomy from VCI by reducing the risks of intraoperative and postoperative complications, through the use of a thromboextractor, which eliminates the risk of intraoperative separation of a tumor thrombus.
