METHOD OF LAPAROSCOPIC RADICAL NEPHRECTOMY WITH THROMBECTOMY FROM INFERIOR VENA CAVA Russian patent published in 2024 - IPC A61B17/00 A61B17/12 

FIELD: medicine; oncourology; abdominal surgery.

SUBSTANCE: renal artery, a kidney within Gerota fascia is separated by the first stage. Renal artery is clipped and cut off. At the next stage, the inferior vena cava is separated below the renal vessels, then the inferior vena cava is separated up to the level of short hepatic veins. Above the level of renal vein discharge, at the level of short hepatic veins, the inferior vena cava is allocated in a circular manner. At this point of the inferior vena cava, a vascular tourniquet with length of 70 cm is inserted under the inferior vena cava, and both ends of the tourniquet are brought out through an additional port for laparoscopic surgery under the xiphoid process. Thus, it is possible to temporarily clamp the inferior vena cava by tightening and fixing the tourniquet through the installed silicone tube. With this method, the blood flow in the inferior vena cava is temporarily closed from the external medium without using vascular clamps.

EFFECT: method enables avoiding the vascular clamp in the area of short hepatic veins, eliminating the risk of intraoperative damage to the inferior vena cava during the manipulation; to perform temporary occlusion of the inferior vena cava at any moment of the operation even without visual control of the operating picture, and also significantly increases the safety of this stage of the surgical intervention.
