FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to maxillofacial surgery, dental surgery and otorhinolaryngology. 3D cone-beam computed tomography is performed to assess the condition of the bone tissue and the size of the defect. A biological resorbable membrane made of a polymer of lactic acid of a U-shape is modeled congruently to the size of the defect, overlapping the length of the bone defect by 5 mm on both sides. Under conduction and infiltration anesthesia, an incision is made along the crest of the alveolar ridge at the site of the existing bone defect. The alveolar process is skeletonized. A U-shaped membrane is installed so that one vertical part of the membrane is located on the inner side of the alveolar process of the maxillary bone, and is fixed to it with fibrin glue. The second vertical part of the membrane is located on the outside of the alveolar process. In the formed cavity, between the fixed vertical part of the membrane and spaced from it, an augmentate is injected, consisting of crushed autologous bone and platelet mass, until the bone defect is filled. The spaced vertical part of the membrane is fixed with fibrin glue to the outer side of the alveolar process. The biological resorbable U-shaped membrane is covered with a muco-periosteal flap. The wound on the mucous membrane is sutured. After 6 months, a radiation assessment of the quality of the augmentate is carried out with the measurement of the density of the newly formed bone, and the installation of a dental implant.
EFFECT: method allows to eliminate post-traumatic and acquired defects of the upper jaw with subsequent restoration of the dentition, to eliminate extensive defects of the alveolar process of the maxillary bone, to avoid repeated surgical intervention.
