FIELD: medicine.

SUBSTANCE: invention relates to medicine, specifically to ophthalmology. For the purpose of treating massive submacular hemorrhages accompanying age-related macular degeneration, performing the three-port subtotal vitrectomy 25 G with removal of the posterior hyaloid membrane. That is followed by intramuscular introduction of 3,000 IU 0.3 ml of Hemase 12 hours before the operation. Removal of the posterior hyaloid membrane is followed by local removal of the macular area of the internal border membrane in the upper temporal quadrant intraoperatively. Further, within hemorrhagic retinal detachment, subretinal introduction of 0.3 ml of 3,000 IU of Hemase and 0.2 ml of sterile air is performed, irrigation fluid is replaced with sterile air by 70 % of volume of vitreal cavity, 0.05 ml of an Eilea angiogenesis inhibitor is introduced intravitreously, a tamponade of the vitreal cavity of 20 % sulphur with SF6 hexafluoride is performed. After preoperative administration of Hemase patient is placed in position “horizontally on back” for 2 hours, and in early postoperative period of patient is placed in position “horizontally on back” for 2 hours, and then transferred into vertical position.

EFFECT: method enables minimizing traumatisation of the photoreceptor layer of the retina in the process of its separation from the blood clot in conditions of accelerated lysis of the fibrin clot, complete dislocation of subretinal blood from central parts of retina in lower eye fundus, in connection with which visual functions are restored to initial level in conditions of no recurrences of submacular hemorrhage.
