FIELD: medicine.
SUBSTANCE: group of inventions relates to medical equipment and can be used in traumatology and orthopedics for replacement of deformed or destructed cotyloid cavity in primary and revision hip arthroplasty. Complex for hip arthroplasty comprises a cotyloid arthroplasty in the form of a cup for replacement of cotyloid defects with a porous outer coating and through threaded holes, as well as spongious screws made with possibility of arrangement in through threaded holes, longitudinal axes of which intersect in point, which coincides with common center of bends of outer and inner surfaces of cup. Cup is equipped with plugs, safety collar and is made with internal circular grooves for distribution of cement mantle, with threaded hole for setting tool and with porous coating having trabecular structure. Holes for spongy propellers are located on the boundaries of the ball layer and are made with internal end grooves for screw heads shaped to deflect the screws by an angle of not less than 15°. At the ends of recesses for screw heads there are threaded holes for plugs. Angle between longitudinal axes of holes at boundary of small-diameter spherical layer for at least 10° greater than the angle between the longitudinal axes of the holes at the boundary of the large-diameter ball layer. Method for installing a complex for hip replacement to replace cotyloid defects involves performing access to the hip joint after preparing the surgical field, treating the true cotyloid cavity with a spherical cutter corresponding to the cup size, making the plastic by compacting the plastic material by reverse rotation of the cutter, cup installation into cotyloid cavity with maximum possible restoration of joint rotation center and fixation of cup with spongy screws, which are blocked with plugs. Spongious screws are inserted at optimum angles through holes in the cup, made with inner end grooves for screw heads, each of which has a shape for deviating the screw from the longitudinal axis through an angle of not less than 15°.
EFFECT: group of inventions prolongs the operation of the acetabular cup in primary and revision hip replacement by optimizing the method of introducing screws and improving the structure of the acetabular cup.
