FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to gynaecology, and can be used to select a therapeutic approach in patients with uterine myoma, including its combination with adenomyosis. Factors of risk of recurrent myomatous growth, such as multiple nodes, moderate anaemia, bacterial contamination of the cervical canal with recurrent bacterial vaginosis, dissemination of the cervical canal and infection of myomatous nodes, moderate expression of Ki-67, moderate or high expression of progesterone receptors, moderate or high expression of vimentin receptors, moderate or high expression of collagen receptors, presence of vascular anastomoses between uterus and ovaries or other vessels, chronic stress reaction, training reaction, hypoactivity, hyperreactivity, endometrial hyperplasia, polymorphism D of gene GSTP1, zero version of gene GSTT1, pre-integrated rehabilitation, diameter of vessels with wall more than 99.6 mcm, zero version of gene GSTM1. Then individual risk of disease recurrence (p) is calculated by declared formula. If the condition is p < 0.5, a low risk of recurrence and subsequent surgical intervention is determined, while if p ≥ 0.5, a high risk of recurrence and a tactic including extended preoperative preparation are determined. That is followed by selecting a surgical approach: if interested in pregnancy - in favour of laparotomy, and if disinterested in pregnancy - laparoscopic removal of clinically significant nodes. That is followed by extensive rehabilitation.
EFFECT: method enables specifying the therapeutic approach to uterine myomatosis and its combination with adenomyosis, reducing the length of patient rehabilitation and reproductive function recovery after myomectomy, improving the quality of life of uterine myoma patients by evaluating the complex of the most significant indicators.
