METHOD FOR DIFFERENTIATED MANAGEMENT OF PATIENTS OPERATED ON FOR DEEP STERNOMEDIASTINITIS Russian patent published in 2025 - IPC A61B6/03

SUBSTANCE: invention relates to plastic, thoracic surgery and X-ray diagnostics and can be used for differential selection of management of patients operated on for deep sternomediastinitis. Method for selecting a management approach to the patients operated on for deep sternomediastinitis consists in performing a bone computed tomography (CT) scan. Diastasis between fragments of sternum and edges of skin-fascial flaps transferred into defect of sternum is determined symmetrically from two sides on axial sections. Diastasis size is determined on four axial sections: at the level of the upper and lower edges of the sternum handle, the upper edge of the sternum body and at the level of the fourth intercostal space. Sternum fusion is evaluated in points on each of the four sections. Further, the points obtained for each of the four sections are summed up, and if total is 5 points or less, a repeated plastic surgery on the sternum is performed; if the total is from 6 to 9 points, a mode of limiting physical activity with dynamic observation and assessment of the condition is prescribed every 6 months; if the total score is 10 points or more, restrictions on physical activity and activity are removed.

EFFECT: carrying out in patients who have undergone plastic surgery of anterior thoracic wall with skin-fascial flaps after deep sternomediastinitis, computed tomography of chest organs with measurement of diastasis on 4-point scale between fragments of sternum and skin-fascial flap in four examined points of sternum and summation of obtained values.
