SUBSTANCE: invention relates to medicine, specifically to vascular surgery. Autoven is separated from extended incisions above pre-marked autovein with sequential dressing and intersection of all tributaries. Autoven is excised in the region of saphenofemoral anastomosis, while the distal portion of the vein is left unbroken, preserving natural blood flow thereon. Proximal portion of the autoven is removed from the wound. Distal anastomosis with popliteal or tibial artery. After applying a distal anastomosis between the reversed autovenous portion, without applying the assembly to the anastomosis, distal end of autovein is cut off and the remaining portion is ligated. In the lumen which is cut off for autovenous shunt, DeBake's needle is inserted. Distal end of autoven is temporarily fixed on olive by ligation on a needle. Syringe is connected to the end of the needle with a mixture of physiologic saline with heparin at temperature of 36–37 °C, and using the syringe, without creating a high pressure, the integrity of autoven is tested and washed from blood residues. Autoven is then inserted along the muscles, a distal anastomosis is applied, and after the blood flow is started, knots are sequentially fixed on a proximal and distal anastomosis.
EFFECT: method enables avoiding early and late complications associated with shunt thrombosis and returning ischemia clinic, recovering working capacity and improving the quality of life of peripheral arterial disease patients after reversible autovein bypass shunt.
