SUBSTANCE: invention refers to medicine, namely to operative gynecology, and can be used for operations in abdominal surgery and oncology. Disclosed is a method for preparing and performing an organ-preserving surgical operation on small pelvis organs using a software and hardware system, which involves the stages of creating a preoperative personified three-dimensional mathematical and graphic model of organs and new growths in them, planning - simulating an organ-preserving surgical operation, surgical intervention with import of a preoperative three-dimensional model creating an intraoperative model. At the stage of planning the operation, the patient undergoes ultrasound examination or computer or magnetic resonance tomography and forms a preoperative three-dimensional model of organs and new growths in them and a surgical navigation surgical plan. Further, the preoperative model during the surgical intervention is adapted to the intraoperative parameters of the organs and new growths and an intraoperative model with surgical navigation is formed. At the stage of constructing a preoperative model of organs and new growths in them, modeling of new growths in organs, which is of interest to the surgeon in the planned operation, is performed. A preliminary plan of operation is then formed to designate navigation marks for the incision and removal of the new growth with minimal trauma of healthy organ tissue. That is followed by an organ-preserving laparoscopic intervention beginning with assessing the topography and size of the organs and new growths therein and adapting the preoperative three-dimensional model to the intraoperative parameters of the organs and new growths, after which, using the software and hardware complex on the video endoscopic complex monitors, it is possible to evaluate volumetric neoplasm in an organ in a dialogue mode, with visualization of invisible implanted structures and vessels. Thereafter, on the adapted three-dimensional model, navigation marks are displayed, with the help of which the place of the incision is positioned, after which the new growth is removed from the organ, in the process of surgical intervention with spatial displacement of organs or change of their geometry due to removal of new growths, model reattachment occurs automatically at fixed time moments, or manually by means of computer control system.

EFFECT: invention enables removal of small pelvis neoplasms with minimal trauma of healthy organ tissue requiring no or minimal application of coagulation techniques for hemostasis; in ovarian operations the technical result is also to preserve healthy ovarian tissue and ovarian reserve.
