FIELD: medicine.
SUBSTANCE: invention relates to otolaryngology. Surgical intervention with interstitial laser exposure is performed. Semiconductor laser light wave of 0.6 mm is submucosal in a region of a front edge of an inferior nasal concha. In the retrograde direction, continuous contact laser coagulation of cavernous venous plexuses of each of the inferior concha is performed with formation of a tunnel between the bone and the mucosa of the shell along its entire length. At all stages of the surgical intervention, video endoscopy is carried out using an endoscope with a viewing angle of 0°. Tunnel is formed by performing instrumental submucous vasotomia by inserting elevator-dissector through anterior incision of inferior nasal concha. It is moved along the bone segment of the concha by separating the mucous membrane along the entire length and width of the medial surface. That is followed by lateroposition of the nasal cone displacing the skeleton of the cone in the lateral direction by means of an elevator raspatory. That is followed by visually evaluating the mucous membrane volume reduction. That is followed by laser exposure, moving the light guide fiber along the entire length of the formed tunnel to the posterior end of the inferior nasal concha, exposed to a laser at wavelength 980 nm, radiation power 4.0 W for 15-20 seconds. Volume of the posterior end of the inferior nasal concha is visually evaluated with a simultaneous increase in the width of the nasal passage and widening of the lumen of the posterior nostril. Thereafter, the fiber of the light guide is moved backwards from the back end of the shell outside to the inside of the formed tunnel at rate of 2.0-3.0 mm per second, performing laser coagulation of vessels and cavernous plexuses of the inferior nasal concha within the upper, medial, lower edge of the shell. That is followed by controlling control of the volume of the inferior nasal concha.
EFFECT: method enables reducing a risk of intraoperative and postoperative complications, as well as recurrence of disease due to characteristics of laser exposure with reduced volume of inferior cavities with expansion of common nasal passage to improve nasal ventilation function without damaging surface epithelium of shell and excessive carbonation.
