FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopedics, and can be used for rational perioperative management of patients with periprosthetic infection in the region of the hip joint when performing sanitation surgery with removal of the endoprosthesis and installation of the antimicrobial cement spacer. Method involves draining a postoperative wound and pharmacological prevention of venous thromboembolic complications by anticoagulants in doses indicated for orthopedic operations. Drainage is performed within one day after surgical intervention. That is followed by performing the puncture of the operated joint on 3rd and 7th days. First introduction of low-molecular heparin is performed not earlier than 12 hours after the operative intervention and then every 24 hours for 4 days. From 5-th day, direct oral anticoagulants are prescribed in standard doses of up to 35 days.
EFFECT: method provides reliable and significant reduction of total and postoperative drainage blood loss, preserves high thromboprophylaxis, reduces the need for allogenic blood transfusion and prevents the local concentration of antibacterial agents in the postoperative wound by reducing the loss thereof with a drainage detachable.
