FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used for treating injuries of the Achilles tendon. Approximation and adaptation of the ends of the ruptured tendon is performed. If it is possible to perform closed approach and adaptation of the ends of the ruptured tendon, the therapeutic action is performed by giving the knee joint flexion to angle of 140–150°, and in the foot - maximum plantar flexion, controlling process of approach and position of tendon ends percutaneously by ultrasonic examination. If closing of tendon ends is not possible, surgical exposure is performed by cutting skin and subcutaneous tissue along posterior external surface of shin in length of 10–12 cm, exposing tendon and aligning their ends with application of tendon suture. On 14th day after approaching and adapting the ends of the broken tendon by injection, the mixture is introduced once with mesenchymal stem cells previously isolated from autologous biological tissue and cultivated in amount of 4.7⋅107 cells/ml. In the postoperative period, a gypsum immobilisation of the injured extremity is performed in the position of flexion of the shin and foot from the finger tips to the middle third of the thigh for no more than 4 weeks. Autologous biological tissue is represented by adipose tissue taken from subcutaneous fat of a patient from an external lateral surface of his thigh in a projection of a greater trochanter with closed approach and adaptation of the ends of the broken tendon, and in surgical treatment using adipose tissue taken from the same localization, or using adipose tissue obtained from operational access. Mixture with mesenchymal stem cells is introduced into a tendon injury area in amount of 10 million cells in 10 ml of 10 % albumin solution.
EFFECT: method prevents formation of scar-connective tissue in the Achilles tendon injury zone at least 6 weeks after the introduction of a leukocyte concentrate containing mesenchymal stem cells, as well as provides reduced traumatism due to rejection of bone marrow sampling.
