METHOD FOR PROGNOSTICATING DEVELOPMENT OF LOWER LIMB ISCHWMIS DISEASE Russian patent published in 2021 - IPC A61B5/00

Abstract RU 2745327 C2

FIELD: medicine.

SUBSTANCE: invention relates to the field of medicine, in particular to methods for prognosticating clinical outcomes in patients with critical lower limb ischemia (CLLI) on the basis of occlusive artery diseases. At present the high frequency of lower limb amputations makes the problem of forecasting the reversibility of the ischemic process and the curability of patients with CLLI particularly relevant. A significant improvement in the quality of treatment of patients with lower limb ischemic disease (LLID) can be achieved by providing a timely and fairly accurate prognosis of the development of the studied disease at various stages of patient management. The essence of the invention is that, using generally accepted medical techniques with the use of a tonometer and an apparatus for ultrasound Doppler studies, the ankle-shoulder index (ASI) – Y1 is measured, and the rheographic index (RI) – Y2 is determined using a rheograph, by palpation or with the help of devices for recording electrophysiological parameters, the heart rate (HR) – Y5 and the respiratory rate (RR) – Y4 are determined, and the ratio of the peak systolic velocity in the stenosis zone to the velocity in the prestenotic zone (K=Y8=PSVUSDG) is determined using an ultrasound tomograph. Using laboratory analyzers, D-dimer - Y6, activated partial prothrombin time APPT - Y7 are determined, and the quality of life QL - Y9 is determined using the following questionnaire: a) the limitation of pain-free walking distance (less than 20 meters) – 1 point; b) pain on any exertion, but no pain at rest – 3 points; C) pain at rest after 5 hours of sleep – 4 points; d) pain at rest after 2 hours of sleep – 5 points; d) constant pain without swelling feet – 6 points; e) persistent pain, swelling of the feet with skin hyperemia – 7 points g) persistent pain, swelling of the feet with skin hyperemia, acrocyanosis of the feet – 8 points; h) constant pain, swelling of the feet with skin hyperemia, acrocyanosis, trophic ulcer of the foot is 10 points.

EFFECT: invention improves the quality of prognasticating the development of the severity of the lower extremities ischemic disease by taking into account the degree of decompensation of the system of regulatory organs: heart-brain (H-B).
