FIELD: medicine; anesthesiology.

SUBSTANCE: invention relates to medicine, namely to anesthesiology, it can be used for combined anesthesia of patients undergoing lung resection. To do this, after adequate prooxygenation, induction into anesthesia is performed by intravenous bolus injection of propofol 1.5-2 mg/kg, rocuronium bromide 0.5 mg/kg, fentanyl 2-3 mcg/kg, lidocaine 1.5 mg/kg, ketamine 0.5 mg/kg. After intubation of the trachea and main bronchus with a two-light tube, anesthesia is maintained in the intraoperative period by inhalation of 0.6-0.7 MAC sevoflurane and continuous infusion of lidocaine at a dose of 1.5 mg/kg/h. 5 minutes before the start of the operation, an additional 100 mcg of fentanyl is injected. At the end of the operation, the trachea is extubated on the operating table and the patient is transferred to the intensive care unit with a continued infusion of lidocaine at a dose of 1.5 mg/kg/h during the first 24 hours of the postoperative period.

EFFECT: method allows, with minimal impact on hemodynamics, reducing the severity of postoperative pain of thoracic patients with the same degree of intensity and without additional invasive manipulations, as with a combination of general and regional anesthesia.
